Doctors Reveal the Surprising Causes Behind Heavy Legs

It’s very common for your body to feel sore and tired after a long and busy day, regardless of your age. But sometimes these feelings, such as when your legs feel particularly heavy, may signal an underlying health condition that needs addressing. That’s why we asked doctors what it means when your legs feel heavy—and it turns out, there are a handful of different reasons.

Meet the experts: Kevin Kheder, M.D., an interventional cardiologist at Hoag’s Jeffrey M. Carlton Heart & Vascular Institute in Newport Beach, CA; Christopher Yi, M.D., a vascular surgeon at MemorialCare Orange Coast Medical Center in Fountain Valley, CA.

“People often describe heavy legs as feeling tired, weighed down or difficult to move, almost as though they’re walking through mud,” says Kevin Kheder, M.D., an interventional cardiologist at Hoag’s Jeffrey M. Carlton Heart & Vascular Institute in Newport Beach, CA. “The sensation may be accompanied by aching, cramping, swelling, or fatigue in the legs, particularly after standing for long periods.”

Read on to find out the reasons why your legs might be feeling heavy, plus when you need to act and when the feeling might signal something serious.

Common reasons for heavy legs

“There are several reasons you might experience heavy legs,” says Christopher Yi, M.D., a vascular surgeon at MemorialCare Orange Coast Medical Center in Fountain Valley, CA. Here are the ones that doctors say you should know.

Chronic venous insufficiency

“One of the most common reasons is chronic venous insufficiency, a condition where the veins in your legs have trouble sending blood back up to the heart,” Dr. Yi explains. Per the Cleveland Clinic, chronic venous insufficiency is quite common, affecting one in every 20 adults. In addition to heavy legs, it can cause burning or tingling in the legs, nighttime leg cramps, swelling in the lower legs, and varicose veins, among other symptoms.

Acute lifestyle factors

Simple, everyday things, such as sitting or standing for too long, muscle fatigue after intense exercise, or dehydration can make your legs feel heavy, Dr. Yi says.

Carrying extra weight

Dr. Yi notes that if you’re overweight or obese (or if you’re pregnant), your legs may feel heavy, especially after activity. Why? Carrying extra weight increases the stress on your weight-bearing joints, such as your ankles, knees, and hips, per the Mayo Clinic, plus promotes inflammation that can cause aching, fatigue, and heaviness.

Peripheral artery disease

“In some cases, heavy legs may be related to peripheral artery disease, nerve problems, or other medical conditions, especially if symptoms occur while walking and improve with rest,” says Dr. Kheder. According to the Mayo Clinic, peripheral artery disease is a condition in which the arteries become narrower, reducing blood flow to your limbs. Aching, heavy legs may be a symptom, along with (but not limited to) painful cramps in the hips, thighs, or calves while walking, coldness in the lower leg or foot, and leg numbness.

Certain medications

Some prescription drugs count muscle weakness, fatigue, and stiffness among their possible side effects, which Dr. Kheder says may contribute to feelings of heavy legs. Per research in Biomedicines, these include statins, beta-blockers, steroids, antimalarial drugs, some cancer drugs, gout medication, and HIV medication, among others.

What to do if your legs feel heavy

“If the feeling of heaviness comes and goes, simple lifestyle changes can often bring relief,” says Dr. Yi. “Try elevating your legs when you rest, staying active throughout the day, and taking short walks to keep blood flowing. Compression stockings can also help support circulation. Staying hydrated and avoiding long periods of inactivity are small habits that can make a big difference.” If these changes help keep the heaviness at bay or banish it quickly, there’s likely nothing serious behind the feeling.

That said, if heavy legs become a frequent, persistent issue, the feeling gets worse, or it starts to interfere with your ability to carry out your regular activities and routine, it’s worth making an appointment with your doctor. “Seek immediate medical attention if one leg suddenly becomes swollen, painful, warm, or red, as this could indicate a blood clot that requires urgent treatment,” says Dr. Kheder. “Likewise, sudden leg pain with loss of color or a cold foot warrants emergency evaluation because it may signal an acute problem with blood flow.”
